What you'll do

• Provide comprehensive care to patients with acute conditions
• Manage patients with chronic medical conditions
• Address mental health and addiction-related medical conditions
• Conduct patient assessments and develop treatment plans
• Deliver care in community health setting
• Serve the Scarborough community
• Maintain clinical documentation and electronic health records
• Collaborate with multidisciplinary healthcare team

What you'll need

• Nurse Practitioner licensure/registration
• Valid provincial license to practice in Ontario
• Experience in primary care or community health settings
• Ability to manage diverse patient populations
• Full-time availability, Monday-Friday
• Willingness to work in Scarborough community
