What you'll do

• Deliver speech and language pathology services to clients
• Conduct patient assessments and evaluations
• Develop and implement treatment plans
• Provide therapy in home and community settings
• Maintain patient documentation and clinical records
• Collaborate with integrated care team
• Support compassionate community care model

What you'll need

• Speech Language Pathology certification/licensure
• Valid professional credentials
• Home and community health care experience preferred
• Ability to work as independent contractor
