Position Summary:
The Case Manager is responsible for the overall coordination and management of client care, from initial assessment to ongoing service delivery. This includes developing individualized care plans, coordinating caregiver schedules, conducting regular client visits, and ensuring compliance with all regulatory requirements. The Case Manager also acts as a liaison between clients, families, caregivers, and other healthcare professionals.
Responsibilities:
- Conduct initial client assessments to determine care needs and develop individualized care plans in collaboration with the client, family, and other healthcare providers.
- Coordinate and manage the scheduling of caregivers to meet client needs, considering caregiver skills, availability, and client preferences.
- Hybrid role - Weekly/Bi-weekly client visits to monitor the care requirements.
- Regularly monitor client satisfaction and the quality of care provided through home visits, phone calls, and feedback surveys.
- Supervise and evaluate the performance of nurses and caregivers, providing ongoing support and training as needed.
- Address client concerns and resolve any issues related to care delivery.
- Maintain accurate and up-to-date client records, including care plans, progress notes, and communication logs.
- Collaborate with other healthcare professionals, such as physicians, nurses, and therapists, to ensure coordinated care.
- Stay informed about relevant regulations and best practices in homecare.
- Participate in case conferences and team meetings.
- Manage a caseload of clients effectively and efficiently.
- Participate in the recruitment and onboarding of new caregivers.
- May be required to perform on-call duties.
Qualifications:
- RN/LPN with a current BCCNM registration (required)
- Cardiopulmonary Resuscitation certification/CPR (required)
- Minimum of 2-3 years of experience in community nursing, home healthcare or a related field.
- Experience in case management, care planning, and caregiver supervision.
- Strong knowledge of homecare regulations and best practices.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a team.
- Proficiency in using electronic health records (EHR) or other relevant software.
- Valid driver's license and reliable transportation.
- Ability to pass a background check.
Schedule:
- Day shift
- Evening shift
- Monday to Friday
- Weekends as needed
