What you'll do
- Actively seek out individuals in the community who are homeless or at risk of homelessness
- Connect individuals with appropriate resources and services
- Apply psychosocial rehabilitation principles in all interactions
- Maintain alignment with Coast Mental Health mission, objectives and policies
- Conduct outreach activities
- Document client interactions and progress
What you'll need
- Understanding of psychosocial rehabilitation practices
- Ability to work variable schedules
- Reliable transportation or transit access
- Background in social services or related field preferred
- Knowledge of homelessness and mental health issues
- Valid driver's license (may be required)
