What you'll do
- Provide pharmaceutical care to clients in home settings
- Manage and optimize medication therapy
- Conduct clinical assessments
- Collaborate with interdisciplinary healthcare teams
- Support client independence at home
- Deliver care across homes, schools, and retirement homes
What you'll need
- Pharmacist license/registration
- Home healthcare experience preferred
- Knowledge of Ontario healthcare system
- Ability to work in diverse care settings
